Hi all, i had my review appointment today with my Rheummy. He went through how i was etc and i mentioned my blood results as i was puzzled by one of them. However he re-assured me that all was indeed very well and my liver is coping extremly well with the methotrexate. ThumpUp I last saw my rheummy about 6months ago and i actually saw a locum who announced that i HAVEN'T got RA, but i have Psoriatic Arthritis! I have no symptoms of any skin problems and my origional diagnosis was RA which was 2 years ago. Blink
My Rheummy went through a full check of my joints and my original symptoms and asked if i had or ever had any 1st digit pain/swelling or any nail pitting or splitting. I haven't and never did have so i am puzzled.

He said it would be a good idea to get a proper official diagnosis to see if i have indeed RA or Psoriatic Arthritis.

The symptoms on 1st diagnosis are very similar in RA and Psoriatic Arthritis. Some people never get the skin problems, some only get the joint swelling/pain problems. The 1st digit pain/swelling is however charicteristic of Psoriatic Arthritis. On my 1st diagnosis i was symetrical with my pain and swelling. I didn't have red hot areas, just boggy fluidy type swellings, pain, stiffness nausea and generally feeling, well awful.Scared

So, as you can see, puzzled!Razz

Just wondered how many other people have had this told to them and i will keep you all posted of my diagnosis!!!RollEyes

Sorry I can't help Dawnybee but haven't come across this before. Would the treatment you get be different for PA? What are they doing to confirm the diagnosis?

Hi Dawnybee.

I have RA- Sero positive, errosive, severe, anti-ccp positive. NOt PA. Do you know what sort they think you might have? Have they done the anti- ccp test?

I get splitting and dry nails. my nails are ridged etc. Its all to do with the RA.

a GOOD rheumatologist looks at my hands. not only the joints. they have a shiney, sallow and slightly waxy appearance.

I'm also a query Psioratic Arthiritis person. I don't have psoriasis though but I do seem to have the first knuckle swelling and ridged nails. I've found however that the consultant is quite happy to disregard all that and just tells me that the treatment is the same anyway.
I get annoyed by that attitude though because i would like to know WHICH type of RA I've got.


Not much to ask surely?
YES I'VE CHANGED, PAIN DOES THAT TO PEOPLE.

Thanks for all your replies. Julie, thats exactly how i feel! It is more or less the same treatment AKA methtrexate (which i take) I don't have the first knuckle swelling or pitted splitting nails. However, it would be "nice" to know exactly which one i have! Jenni, I haven't had my CCP done. I hope that will be the blood test they were talking about. Thanks again, dawny

Hiya all, i am confuzzled too , to a degree .

I am sero positive for Rheumatoid Factor and diagnosed with RA 9 months ago (ish) , yet recently i have had Psoriasis on my legs , lower back and Bum :O.
I also have Thick red skin on my elbows which suggest Psoriasis as well.
My Consultant has put RA/PsA on my MTX book.

Most of the reputable sites suggest that to get a diagnosis of PsA one of the criteria is that you have to have a sero-negative RF.
However i have found a few reports that mention that it is "rare" to have a sero +ve RF AND have PsA. This suggests that there are a few people who have a +ve RF but do not have RA but have a diagnosis of PsA.
Yes the drugs to treat both are the same , however the outcomes and long term effects have different possibilities to RA.

With such a long time between appointments i don't think i will get a quick answer.
I have emailed Consultants secretary to ask the Consultant but again i don't think i will get a definitive answer.
Meanwhile in addition to the 7 prescription items i take , i am using Dovonox , and Betnovate creams on the affected skin areas .. and someone on the Arthritis care site has recommended Aqueous Cream for the dry skin issues .
Only £2.40 (500g) from Tesco .

Well it took them 2 years to get the RA diagnosis , lets hope they make the. minds up soon.

Curiously enough in the call to the Consultants sec..i asked where the write up was of my appointment 8 weeks ago , and was told that she was only writing up January's reports at the moment , as they were understaffed (she did state mine would now jump to the top of the queue.)
RA funding severely lacking here :( and thats how i treat the issue ..as a funding one :)

Gosh! it all gets more confusing! Yes Rich, you're right, the treatments are very similar but the outcomes are very different. I hope you don't have to wait too long for your report to come through. Fortunatly my report/letter comes through quite quickly when i have seen rheummy. I will wait till i get that through the post and see exactly what has been written and then give them a ring. I just got leaflets from the Psoriatic Arthritis Charity . PAPAA and the help there is minimal compared to here or elsewhere .
But when you think there are 100 types of Arthritis and associated conditions then no -one can be pigeon holed into one group or another :)
Sun is blazing here , must be summer .. wish i could get out and enjoy it :)

Rich, although I don't have ps arthritis, I do have dry flaky skin following a bout of Lichen planus, I tried the aqueous cream and didn't rate it much, but the doctor prescribed Cetraben cream which comes in a pump dispenser and you can use it instead of soap to wash in and as a cream. I've found it quite helpful and as it goes on the prescription doesn't cost me (well only the yearly charge) .

Puzzled now. When I last saw my consultant the top joint of my right index finger was swollen and very painful. She told me it must be OA because you don't get RA in the top joint (I found that rather a strange idea -what is so different about that joint?) I also have weak nails which tend to split. Clearly I now need to find out more -something else to worry about!
